Amgaewch dair dogfen bwysig am Brofion Llif Ochrol ar gyfer dysgwyr hŷn yn yr ysgol.

Rydym wedi derbyn cyflwyniad o Lywodraeth Cymru a fydd yn caniatáu i ddysgwyr ym Mlwyddyn 7, Blwyddyn 8 a Blwyddyn 9 gynnal y profion, os rhoddir caniatâd.

Y dogfennau yw:

  • Llythyr gwybodaeth
  • Ffurflen gydsynio
  • Hysbysiad Preifatrwydd

Cyn imi amlinellu’r cynllun, rwyf am bwysleisio nad yw’r profion yn orfodol a’u bod yn gwbl wirfoddol.

Sylwch: ni chyhoeddir pecyn prawf, nes bod ffurflen gydsynio wedi’i derbyn.

Mae’r cynllun fel a ganlyn:

Ffurflenni caniatâd a gwybodaeth a anfonwyd ddydd Gwener, Ebrill 30ain  2021

Ffurflenni caniatâd wedi’u llofnodi i’w dychwelyd i’r ysgol ddydd Mawrth Mai 4ydd, neu cyn gynted â phosibl wedi hynny (anfonwyd y dogfennau i gyfeiriadau e-bost disgyblion i’w hargraffu) – os na allwch argraffu a llofnodi’r ffurflen gydsynio neu ei mewnosod. llofnod electronig, e-bostiwch eich caniatâd i gyfeiriad e-bost y swyddfa: office@llanfyllin.powys.sch.uk

Gellir casglu citiau prawf llif ochrol o’r Dderbynfa.

Mae 3 phrawf ym mhob cit, ynghyd â manylion cyfarwyddiadau llawn a hoffem i’r profion gael eu cymryd ddwywaith yr wythnos ar ddydd Mercher a dydd Sul.

Rhaid rhoi gwybod i’r GIG am bob canlyniad prawf (gweler y wybodaeth mewn llythyr) a dilyn gwefan Gov.uk.

Dear Parents and Carers,

Please find attached three important documents about Lateral Flow Tests for younger learners in school.

We have received a delivery of tests from the Welsh Government which will allow learners in Year 7, Year 8 & Year 9 to undertake the tests, if consent is given.

The documents are:

  • Information letter
  • Consent form
  • Privacy Notice

Before I outline the plan, I want to stress that the tests are not compulsory and are entirely voluntary.  

Please note:  a test kit will not be issued, until a consent form has been received.

The plan is as follows:

Consent forms and information sent Friday, 30th April 2021

Signed consent forms to be returned to school Tuesday 4th May, or as soon as possible thereafter (the        documents have been sent to pupil email addresses to be printed) – if you are unable to print off & sign the consent form or insert an electronic signature, please email your consent to the office email  address:  office@llanfyllin.powys.sch.uk

Lateral flow test kits can be collected from Reception.

There are 3 tests in each kit, complete with full instruction details and we would like the tests taken twice a week on a Wednesday and a Sunday. 

Each test result must be reported to the NHS (see information in letter) and follow the Gov.uk website.

We only need you to notify the school, if the test result shows positive by sending an email to:
