Os nad ydych wedi ei gwblhau yn barod, a fedrwch glicio ar y ddolen isod ac ateb y cwestiynau erbyn Dydd Llun 10fed o Dachwedd os gwelwch yn dda. Os oes gennych ddisgybl ym mlwyddyn Derbyn nid oes angen i chi ei wneud eto gan fod y wybodaeth ddiweddara gennym. Os oes gennych fwy nag un plentyn a fedrwch gwblhau ffurflen unigol ar gyfer pob plentynos gwelwch yn dda.
