‘Os yw’ch plentyn yn rhan o gynllun Profi Llif Unffordd yr ysgol ar hyn o bryd ac yr hoffech iddynt barhau i brofi yn y tymor ysgol newydd, mae citiau prawf ar gael i’w casglu o’r Dderbynfa Ysgol ddydd Mercher, 1 Medi 2021 – bydd y dderbynfa ar agor rhwng 9.00am-3.00pm.
Os nad ydych wedi cofrestru’ch plentyn i gymryd rhan yn y cynllun profi ond yr hoffech nawr iddynt ymuno, unwaith eto, gellir casglu citiau fel y manylir uchod ond mae angen ffurflen ganiatâd wedi’i chwblhau cyn i’r pecyn gael ei gyhoeddi – gellir llenwi’r ffurflen hon pan fyddwch chi codwch y pecyn prawf. ‘
Message for parents/carers of pupils in year 7 and above:
If your child is currently part of the school’s Lateral Flow Testing scheme and you would like them to continue with testing in the new school term, test kits are available to be collected from School Reception on Wednesday, 1st September 2021 – Reception will be open between 9.00am-3.00pm.
As before, the tests should be undertaken every Sunday and Wednesday, so the first test is due to be taken Wednesday, 1st September ready for the start of term on Thursday, 2nd September.
If you have not registered your child to participate in the testing scheme but would now like them join, again, kits can be collected as detailed above but school needs a completed permission form prior to the kit being issued – this form can be completed when you pick up the test kit.
